What's the best incision to use for breast enlargement?
There are several types of incisions that can be used for breast enlargement. Each incision has its advantages and disadvantages. Frequently, the type of incision used depends on the surgeon's experience and preference.
One of the most commonly used incision techniques in breast enlargement surgery is an inframammary crease incision. This incision is made in the crease where the lower part of the breast touches the chest. When properly designed, this incision falls right in the crease of the breast and is very hard to see. It affords the breast surgeon the best exposure for breast augmentation and is associated with the lowest incidence of implant infection and numbness of the nipple.
Another common technique is the periareolar incision. This incision is made around the lower border of the areola, which is the darker skin that surrounds the nipple. This approach cuts through some of the breast ducts and is associated with a higher risk of implant infection. It is generally not possible to use this incision in people with areola diameters less than 3.5 cm.
Another technique is an incision in the armpit (transaxillary). While this sounds like an ideal place for the incision, unless the scar heals perfectly, it is visible whenever you wear something sleeveless. Since the nerves to the nipple course through the axillary area, this approach is associated with a higher incidence of numbness of the nipple. The axillary area is hard to sterilize and this approach will have a higher chance of implant infection as compared to the inframammary approach.
In the transumbilical approach, the so-called TUBA procedure, an incision is made in the upper boarder of the umbilicus. The breast implant is then pushed through a tube into the breast. In this approach the implant can only be placed above the muscle. It is very difficult to control the implant position and there is a high rate of implant rupture from the trauma of inserting it through the insertion tube. For these reasons, it is not currently considered a recommended technique.
